“Fired” isn’t a word with a strict legal definition; while the “out” part of “up or out” meets the conceptual definition of being fired in the sense that your company tells you you can’t work there anymore, most people make a distinction between being “counseled out” (meaning there’s no path to promotion and you therefore have to leave) and being “fired” (like if you get caught committing expense fraud or sexually harass someone). This isn’t a strictly enforced distinction, but it’s one that consultants and other folks in up-or-out industries commonly make.
The actual process can vary a lot, and depends heavily on the mechanics of how the talent management process at a given firm works. Many firms will help you find the next role, since finding good exits for people who get pushed out is a critical part of keeping the funnel full at the front end.

Unless you are fired for cause or there are layoffs, HR and your Partner will eventually tell you in a conversation that you're not going to make Partner which is when these conversations generally happen. Usually you have a few months or so of continued employment and can look for a job.
