You put your best guess initially. Then they’ll call you with more screener questions with a little more info (like if you said you were a former smoker, they’ll call to get the dates you quit. If you say you were a drug user, they’ll call to see if it’s weed or something else and how often, etc). Those calls will verify your health status for the application. Then the life insurance company will send a nurse out to you (or ask for you to go in to a PCP) where they will ask more questions, take your blood pressure a couple times, and take your blood. They’ll get the name of any specialists you’ve seen in the past 5 years and reach out to them as well. Once all that’s done, they might adjust your health status or approve you at whatever you applied under.

If you up your coverage enough they make you do blood work and / or a physical. I just added another $1.2M of coverage and I had to go in for blood work (but no physical)
