Hang in there! The first step is that you recognize what you are doing and want to change.
1. Try seeing a CBT/DBT therapist. They focus on teaching you skills and behaviors to use in life and for challenges. They can help get you on a good path.
2. You sound stress. Maybe do 1 thing a day that makes you super happy (petting a dog, go on a run, watch a funny tv show, etc) to decrease your stress. Also, ask the therapist about some stress management techniques.
3. Reach out to friends or make new friends to stay busy and connect with them.
4. Plan something to look forward to, whether it’s a call to an old friend, joining a social sports league, going to a new coffee shop, etc.
5. For rent, could you ask a parent or family member to float you the money for a few months? That gives you time to save up some more money or look for a new place.
Sorry you are going through a tough time. :( But you have time to get back on track and you can do it!
Also, if you have depression or anxiety (or something like that, or if the therapist diagnoses you with it), file it as a disability with HR. It’s protected under the disability act, so they can’t fire you for that. (At least that’s advice I’ve heard before)

I think levelling with your boss and asking for their help could actually be one way forward. You’re basically on the road to getting fired, and being honest with them might help reverse that. Especially because in the place you’re in right now, having a job is far better than not. Plus all the stuff above. Hang in there, stuff does get better at some point.
Are you clinically diagnosed with depression? If so, it’s a disability that comes with certain protections. You might qualify for reasonable accommodations, alongside a plan for how you will deliver on expectations. I was once in a state of crisis and worked with HR, my manager, and a psychiatrist to pull myself back up and turn around my performance to be a top contributor. Nothing is finite.
This hurts because this is now leading to burned professional bridges. I can’t even begin to properly look myself in the mirror anymore.
Take a paid leave of absence!!!! Set up time to check in during it with your manager and return after 4 weeks. Give yourself time to hit reset and come back renewed.
Can you talk to your boss and tell them your situation? Tell her you’re struggling mentally, but take pride in your job and don’t want to leave.
My last job had an employee assistance program through a behavioral health provider. It might be worthwhile to talk to HR to see if you have that benefit. It really helped me.
