I’m finding it harder and harder to give my patients the care they deserve with all the corporate policies in place. From restrictive appointment times to mandatory documentation requirements, I feel like I’m constantly battling the system. For example, I only get 15 minutes per patient, which leaves little room for genuine conversations about their concerns. I try to push back when I can, but it feels like a losing battle. How do you all handle these restrictions and still deliver personalized care? Any strategies for advocating for your patients while navigating corporate red tape?

I'm not afraid to challenge the status quo and push back against policies that hinder patient care. I've spoken up at meetings, voiced my concerns to management, and even organized petitions to advocate for change. It's not always easy, but I believe it's our duty to stand up for our patients and fight for a healthcare system that prioritizes their needs.

It’s very hard. The expectations are unreasonable. Without sacrificing quality patient care, practicing good medicine and cya, it results in long hours on the computer, a lot of work at home, and lack of sleep for the provider leading to burn out and thus losing a good provider. It’s a shame the corporate gods or higher ups don’t realize this. But their decisions, and for sure, their loss.

I am trying to practice a version of intuitive/mindful eating - honoring my body’s hunger cues and not sticking to a timeline or plan as to what and when to eat. But tbh years of “dieting” has screwed over my ability to recognize hunger cues. I suddenly realize my hunger after not eating for 10+ hours and then I’m ravenous and would just eat anything. Anyone dealt with this have any recommendations? TIA

We recently got the ok to schedule our orientation and delivery date for our IVF medications and all I have to say is ADVOCATE FOR YOURSELF! My husband has dermatomyositis, and his doctor put off treatment so that we could do some collection and go through IVF. I felt like my dr was blowing off our timeline, but I persisted. I worked with CVS Caremark directly and got approval in 2 days (not the expected 3 weeks). Moral of the story: ADVOCATE FOR YOURSELF!

What do you guys think about apps like BetterHelp and Talkspace? I understand they might have real therapists working on their content, but a lot of the marketing I see is so vague and broad that it feels like a lie. Seems like "This app got rid of my anxiety with a 30-minute automated text questionnaire" is the new "This tea made me lose 20 pounds in two weeks!" I'm a PA in the PNW working for a private group staffing ED's in several hospitals. Recently the group has asked the PA's to take on a couple new roles (triaging and rapid track from triage i.e. straightforward ESI 4, 5). With these changes the PA's are considering asking for increased pay in the form of an addition of a COLA (cost of living adjustment) to our normal salary. Just accepted an offer with a psych startup. Not exactly the offer I was hoping for as I’m taking a slight pay cut ($73/hr) and it’s a 1099 position.
