I send some version of the below. I usually try to make it a little more casual and friendly, but you're definitely telling them, not asking. You're providing a service, and this is the cost of your service. They may try to haggle with you (and sometimes I will give very reliable clients a better rate) but this is really a take-it-or-leave-it scenario.
"Hey ________, I'm writing to let you know that my freelance/hourly/daily/whatever rate will be increasing to $XXXX on the Xth of Month. This will/won't affect any ongoing projects, and will be my rate for any upcoming projects.
I hope we can continue working together at this new rate, but please reach out to me if you have any questions or concerns."
I build in a vacation, and when I come back, I say my rates have changed to reflect changing market conditions.
