In person. Always in person if at all possible (they’ll usually ask you to follow up with an email to HR confirming your last day). Two weeks notice is typical (so give notice two weeks before your intended last day), though some people give more if they have a lot to transition. Just thank the firm/employer for all you’ve learned, focus on how the new position fits your career goals, blah blah. You will often be asked where you’re going, it’s up to you to tell them or not (everyone will find out eventually). If you’re going to another firm, some people may be annoyed. If going in house they will love you. It’s rare, but some places will ask you to leave on the spot so be prepared for that as a worst case scenario.
I called my assigning partner first, then HR, then all the partners I regularly worked with. Do it on a Friday afternoon to give everyone the weekend to process (and so your last day will be Friday). Started all of the calls with “Hey, I have some unfortunate news to share and wanted you to hear it directly from me” and it seemed relatively well received.
Friday afternoon, start at lunch to cover all your bases before leaving. Do it in person with key people - main partner, practice group leader, etc. HR can be via email after you've talked to people. Be explicit when you do talk to people "Please don't say anything until I've been able to connect with Person A and B" and then follow up once you have. You need to decide what to say about where you are going - they can ask, but I think it's up to you to decide and practice your response before you get put on the spot. My last departure I just said I was exploring other opportunities and left it at that.
