Legit just take a sick day. No need to specify.
You click the button in your absence tracking system and select “mental health day” (or whatever your system uses). If they aren’t that specific, then select “sick day”.
Then go book an appointment for yoga, a massage, or whatever else helps you get back in the right head space.
That’s what sick days are for. Mental illness is being sick. And mental illness isn’t only just “craziness” or a mental disorder, mental illness can be as simple as being burned out. Your mind is a part of your body, take care of it.

I’m sick I am going to the Doctor. You don’t have to tell them it’s a psychiatrist. People discriminate against the mentally ill.
I would say that you're taking a day out for medical reasons. You don't ask for this, you inform your manager that you are doing this, or you do it quietly on the company's PTO tracker. At this point, seeing a therapist or taking a quiet day is not that different from seeing a doctor or spending a day in bed to recover. Be well.

Depending on where you work most states/cities will have legal “sick days” that are protected employee days.
Use them and call in sick.
