I've had a hybrid role for the past 4 years where I get unlimited sick days (well 14 really until fmla kicked in), 3 weeks of vacation, 3 personal days. I am also given the week after Xmas off. if my toddlers are sick I can work at home with them. Say it’s a question that’s somewhat unusual that you haven’t heard before.
Then ask what is the rationale for asking that question
I would definitely ask this.

Meaning the company is not a good one to join.

Yellow flag. I’d follow up with policy on requesting PTO and how much runway is needed

Definitely a yellow flag. Why does the recruiter need your PTO plans for the next year? Hell, I don’t know my own plans for the next year.
I can see them asking if you have any PTO planned during the first X weeks of employment, if they’re hiring you for a specific project that needs you at that time. But they would need to give you a start date first.
Otherwise, what’s the implication here? You have to request PTO 12 months out?

I would ask what the deadlines and deliverables are for the next 12 months. They will say it has not been determined and you will its the same with your vacation plans.
I feel like this is a major liability for the recruiter… what do they do if you say “I’m having a kid in 6months and will be taking 2 months of FMLA”
They were burned …. just tell them nothing confirmed or scheduled
+1 this
Never heard of that
Major red flag. I've never been asked that, and usually don't have all my trips planned that far in advance, but you better believe I plan on taking some. I've also joined companies where I had planned vacations within weeks of joining. I told them up front, and it wasn't an issue.
I’d offer the next month or two after start date and say nothing confirm beyond that.

Yes. RUN.
