It depends on whether or not it is required for your job. Like at my job it is required if we are on the company health plan.
Since it's not a requirement I think it's strange that they are sharing it, but I'm not sure what the harm is in them sharing it either.
I think this is weird to bring your boss into it. This should be a conversation that they have directly with you.

It doesn’t seem inappropiate as long as that is all they shared. Like, no details, just the fact that you completed the yearly health assessment or not.
They provided names and birthdate and I couldn’t make out the rest. It’s wrong on many levels. They should reach out to the employees not tell their boss. A general “please remind your employees to complete their health assessment if they are taking our medical insurance” would be appropriate but they did not have to provide names and birthdates.
