Do you really want to combine accounts? Or just pool points for a one-time transaction? I did that a few years ago and basically they made us fill out some kind of a form to transfer X points to one account and then that account makes the reservation.
And think seriously about whether you REALLY want to combine accounts. This is your BF, right? Things happen. Heck, even my husband and I have a paragraph in our prenup about points and miles being excluded from the list of community assets. And no, my lawyer said it wasn't the strangest clause he's heard of.
Believe both accounts need to be associated with the same mailing address
