I just did, for several weeks. It was very hard for me to admit that I needed it, and it was the best thing I've ever done for myself professionally. I used a combination of vacation, sick leave (at full pay) and short-term disability (meaning I was paid a fraction of my salary by the state for some of the time). My doctor wrote a note supporting the leave so it was medically-recommended. Feel free to DM if you want to hear more.
Have you talked to your therapist or doctor about Short-term Mental Health Disability Leave (MHDL)?
If you’re going to take more than 2 weeks of PTO, I’m pretty sure you need the documentation for medical leave.

I’m in this same position. It’s scary and confusing
I did. In my case, I was really depressed. I had an interview with someone who determined if medical leave was appropriate (I can't remember who this was, someone from the insurance company maybe?). They definitely wanted to ensure it was real -- I couldn't just say I was burned out or stressed, I had to convince them I was truly sick. But once I did, they granted me six weeks off.
It was a huge help. But, I wasn't 100% better in just those six weeks. My boss was not great, she expected me to be back to chipper and cheerful the day I returned. So the time off was good, but coming back undid a bit of the progress.
