If your resume is up to date, they will know you are no longer with your most recent employer. Don't say anything about the how or why you left your most recent employer until a recruiter or interviewer brings it up.
That depends on the situation. Were you terminated for performance issues, laid off, or voluntarily resigned? That context matters for how you frame the communication.
You don't. They are not supposed to ask. They are supposed to call your last job, and ask are you eligible for rehire? If they do that? I think that is standard protocol. Then maybe in the interview they may mention it. At least that is how it was done in the 2000s.
