I'm facing a gut-wrenching dilemma at work, and I could really use some guidance.

A patient with a terminal illness is requesting a treatment that I believe is not only futile, but could actually worsen their quality of life in their final days. However, their family is adamant that we proceed, citing religious beliefs and a hope for a miracle.

I'm torn between respecting the patient's autonomy and my own professional judgment. I want to honor their wishes, but I also can't ignore my conscience and the potential harm this treatment could cause.

Has anyone else encountered a similar situation where you had to balance patient autonomy with medical ethics? How did you navigate that conversation with the family? How did you ultimately make your decision?

Can anyone offer advice for helping with adderall side effects? I was diagnosed a few months ago and I am new to medication. I have been taking 20 mg xr in the morning but have a terrible crash around 2/3pm each day where I just get very very tired. I accepted a FADP offer but I have lifelong crippling anxiety It leads to (TMI) diarrhea, I can’t sleep at night, among a lot of other issues. I see a therapist and have their support in school accom. I appear high functioning and ever since uni went remote my life and performance has only improved. I know I’m capable of getting the work done and they must see potential if they hired me. But I feel that if I ask for WFH or other accom. I’m gonna be shooting myself and my career in the foot.

Hello, I am working in HCL Technologiesas L1 resource my CWL is nagpur but I want to change my working location . It was changed a few months back but they have referred back to nagpur again saying because of some policies so I asked for a project change thought it may be useful for location change, it's been 23 months in my current project I asked for release but my sdm is not willing to release. Outside counsel has been prepping company witnesses for a trial in August. It is my subject area so I am supervising outside counsel. I did not, however, choose the counsel. He is doing a really awful job. Co counsel is better, but not great either. Higher ups love the firm… I do not want to micromanage, but I have genuine concerns about outside counsel’s ability to try this case. Hello all,

I'm a PA in the PNW working for a private group staffing ED's in several hospitals. Recently the group has asked the PA's to take on a couple new roles (triaging and rapid track from triage i.e. straightforward ESI 4, 5). With these changes the PA's are considering asking for increased pay in the form of an addition of a COLA (cost of living adjustment) to our normal salary. Was wondering if any other PA's in the ED have these in their contracts and at what rate.
