One of my managers kicked me in the foot because I was sitting in the main office talking to a coworker and forearmed me in the back because I wasn’t moving fast enough through a doorway. 
…uh, what? I hope you know this isn’t just “bad leadership.” This is borderline assault. This manager would be immediately fired at my organization.
Denying reality and thinking that yelling and intimidation will produce favorable results.
Dishonesty, which tends to manifest in behavior best described as squirrelly. And your comments about people thinking of themselves as leaders ring true. There are a lot of people who learn management or leadership, but that can be shallow knowledge that doesn't really work in the real world.
From my last job's experience with a toxic strategy manager: constantly, and only using the one example of a project with tangible legacy results that's more than 4-5 yrs old experience rather than not only using much more recent examples but in contrast, borrowing (and crediting) other colleagues' *good* examples to show what lessons learned THEY got from observation and sharing it in a good light.
To me it shows humbleness and deferment of personal ego to speak positively of other's experience(s) as well as one's own and thereby lifting the whole team/organization up together. In addition it shows that manager's/leader's savviness/intelligence to learn from other ppls mistakes/successes, incorporate into their own, AND share the lessons for their teams benefit to learn from as well as leading by positive example. Eventually, the positive use case will go back to the original experience owner and if it was done well plus that person is of similar mind, will do the same.
Maybe this is naive of me as it highly subjective to ppls' ego but I try to live by it and if I work with decent people, it's done well for me and my team.
