I use TeacherMade.com and have them report the time when they start & report the time when they end. Then I require them to upload their work for the test.
TeacherMade doesn’t have a timer feature yet, it the above is my workaround. However, they don’t know that, I just tell them that’s how they keep themselves accountable. I teach HS Geometry and it has been a lifesaver. It basically makes your WS in to a PDF and then you put fillable boxes in where they can type their answers.

I'm face to face, so I just wait until they return and we try to make some time for make-up work. It's tough! 😞
I create the test in Google forms and administer during class time so I can monitor them through classwize during the test. It takes away more class time but adds some element of accountability.
My school is hybrid and students must zoom in and are required to have their camera on. For"pencil and paper" assessments, they must show me that their phone is in the background and their workspace must be visible at all times, then they upload their work. We use Schoology in my district and it has great assessment tools to create questions and allows students to enter their work using an equation editor.
I also use KutaWorks, which uses kuta software assignments, and I assign a different set of questions to each student to prevent cheating. All assessments are multiple choice though and you do need an account. It's about $1 per student.
In all honesty, we are all going through so much during this pandemic that assessing students is my least concern. They are going through enough emotionally and don't need added stress with formal assessments. I allow students to use their resources, without the aid of photomath type apps, since they will find a way to cheat anyway. I also try to give questions that are true/false, find the mistake and explain, as well as "choose all that apply" multiple choice questions.
We give tests through our LMS called canvas. The tests are set up with a beginning and ending time. I also set up the tests so that once they answer a question they cannot go back to prior ones. I also set up a large bank of questions and randomly assign each student a certain number of questions to answer. The students are required to have their cameras on and we can see what each student is looking at on their computer through a site called class policy.
