I wouldn't ask for formal accommodations. There's so much stigma associated with mental conditions, and ADHD in particular is associated with children. People are less likely to understand adult ADHD than something like depression.
My preference has been to make requests of supervisors and colleagues directly, posing it as personal preference rather than attributing to ADHD.
For example, when planning goals or career development with manager, emphasize my desire to change up types of projects. I'd say that I want change-ups to learn more and expand my knowledge base. This is true, even though it's also because I'd get bored and inefficient doing the same thing.
I agree with the comment above. I would atleast put them on notice that you have ADHD because it can protect you from the future. You definitely want to disclose it.
Don’t necessarily agree with not asking for accommodations. If you have ADHD and a diagnosis, I’d talk with your HR partner to get help. I’ve used AskJan (job accommodation network) as a great resource https://askjan.org/disabilities/Attention-Deficit-Hyperactivity-Disorder-AD-HD.cfm
