Look up short term disability
No, you aren't. If the injury physically prevented you from doing the work and your employers wouldn't make accommodations for you to get treatment for it, that's not your fault at all. You can't work if you're constantly in pain. Given the circumstances, I don't blame you for quitting. I hope you get healed up and are able to find a better job opportunity soon.
You should get a ADA application filled out so you know in advance if the employer will accommodate
Nope. Going through something similar at the moment. I just quit because my manager would guilt trip me every time I have her a doctors note and had to find coverage. I herniated a disk, and then got the flu and am just now recovering. Not worth a toxic work environment where they don't accommodate or help you.
