I will admit that I used to be like this. Well meaning but very forgetful. What helped me was my manager sitting down with me and figuring out why I forget things and why I let things fall through the cracks. I was still pretty young, and I really appreciate someone taking the time to help me work through a major gap in my professional life. I can earnestly say I've been a better employee ever since then. Maybe take some time and give them the honest feedback that you feel like their forgetfulness is going to impact not only their career but also the patients that rely on them, and work together to troubleshoot?
I would discuss using tools that will help this person stay accountable and on track. Maybe a simple Task List like on Smartsheet because you or they can set up auto reminders to provide updates or reminders for deadlines, etc.
If you don’t have Smartsheet there’s so many other options! Outlook and office 365 has a “to do” feature that’s similar and can be used.

Use teams to assign a tasks with the expectation that they have to be completed within a certain time. Monitor progress and setup weekly touch base meeting. Celebrate small wins. Catch the employee doing right tasks and not just doing wrong things. After 2 weeks, there should be a turn around. If not start performance improvement plan with the help of HR.
