My general boundary is that co workers/managers/attorneys can add me on LinkedIn, but not other personal social media. I am friends outside of work with one co worker who is on my Instagram but we are both paralegals and knew each other before we worked at this firm. I don’t feel comfortable with people who I have a strictly professional relationship having the level of access to my personal life outside of work
People are social animals. This manager is bound to bring up something they see on your social media to others at work. Even something innocent can be misconstrued. I learned this lesson a long time ago and now have the same policy as P1
If yall aren’t that cool, I’d definitely remove them. It’s so weird to me when they do something like that knowing the “relationship” is completely professional. I have one of the manager’s pets in my requests now and it’ll never be accepted 😂

Without knowing the manager or your relationship with them, I can’t determine if it’s appropriate or concerning, but given your hesitancy/concern with it – do not feel obligated to add her and just ignore it.
If the manager says anything about you not accepting it, just say that you didn’t see it/you don’t really use Instagram.
I just generally have a rule of thumb that I don’t add people to my social media unless we no longer work together.
(As a FYI, you can actually remove somebody as a follower on Insta without them knowing. So it’s what I would do here. 😂)
