Unlimited PTO means that PTO doesn’t accrue, so the company doesn’t carry earned PTO as a liability on their books. That means when you leave no PTO will be paid out.
In terms of using unlimited PTO, most companies leave it to the manager’s discretion. The policies typically include a caveat that you have to be meeting your goals; this is implied even if it’s not directly stated.
Ask about whether there’s a mandatory minimum amount of PTO that needs to be taken. Without any guidelines, unlimited PTO can translate to bare minimum PTO. Since employees didn’t have to “earn” each day, they feel less entitled to take it, and managers have fewer qualms about denying requests.
For context, I once worked at a small startup that attempted to transition from unlimited PTO to defined PTO. The CEO proposed 10 days including sick time. That’s what a reasonable amount of PTO was to him. He eventually relented, but unlimited PTO was never truly unlimited there.
