Are you on the high deductible or Open Access Plan?
If HDHP, then the refraction is not covered at all, but the visit with the doctor is covered. The refraction is usually about $50-65.
If the Open Access, then the refraction should be covered once every two years along with the visit. However, MOST plans do not cover the refraction, so don't be surprised if the doctor requires you to pay their standard cost upfront. Make sure they do submit the refraction to insurance though.
