I wouldn't consider Amex payments performance issues. Also, I would have to believe you were contacted multiple times by both automated (from Amex and Internal systems) and by HR directly to pay an outstanding balance that was several months past due. Would also assume your partner was CC'd on an email from HR notifying you to pay it. In that case, the non-action on your part speaks towards other aspects of your work which isn't good. Sounds like your more of a liability than an asset...As recommended above, change that for 2017. Good luck.

A PIP for your AMEX? You had to have some seriously delinquent fees or your firm is extremely hardass these days
You may need to be put on a PIP first
You'll either get put on a PIP or get fired. My guess is fired if you're the only one there since they can PiP you on a phone call.
